The exception surface. Where a held gate lands: symptom traced to proximate
cause traced to originating event, with an action trail that is appended, never overwritten.

The money side, in depth. Funding feasibility, rail selection with an explicit
finality class, and an ISO 20022 instruction package emitted against the published schemas.
Custody is the console, not a tab. Securities leg and cash leg both
run the Pipeline’s gates; the Cash Leg tab is where the money side is resolved to a rail, a
finality class, and a message. Breaks Workbench is orthogonal to both — either leg feeds it.

There is no submission path anywhere in this system, and is_submission is pinned to
False at the type layer so that one cannot be added by accident.
